Licensed Practical Nurse - LPN Essential Functions:
Conduct Home Health Aide skills trainings, competency evaluations, HHA introduction and 90-day evaluations
Complete initial client assessment, plan of care, 90-day reassessments, and ongoing evaluations of the GAFC setting
Partake in both care plan creations and reviews, along with discharge plans
Assess and address motivational, psychosocial, and environmental concerns
Maintain and update client charts, medical records and submit all relevant information to authorized organizations
Completing notes corresponding with each on-site visit or encounter, or more often as the member's condition warrants
Document health changes and communicate to the client's Case Manager and Program Manager in a timely manner
Review all paperwork and plans of care to ensure program compliance, updating information as needed
Performs any other additional duties that may be required by management at any time
Licensed Practical Nurse - LPN Requirements:
Travel is required
Valid driver's license and reliable transportation required
Licensed Practical Nurse - LPN Massachusetts license required
Minimum of 1 year experience in an acute or home health care setting or comparable work experience preferred
Fluency in both English & second language preferred
Case management, social services, and/or community health worker experience preferred
Past experience working with behavioral health population is a plus
Strong communication, writing, organizational, and time management skills
Solution oriented with the ability to multitask
