The Direct Care Worker/Medication Technician is responsible for the daily supervision and care of the client as determined by each client’s treatment plan. The ideal candidate must be able to assist clients who need partial to total care as well as the ability to complete daily medication passes, housekeeping tasks, and preparation of meals while providing excellent direct care to patients.

Minimum Qualifications:

Education: High school diploma or GED

Experience: Prefer at least one (1) year experience with the TBI population or rehabilitation.

Current valid driver's license with no more than (3) points or a valid state ID.

Must be able to pass a drug test

Negative TB test result needed prior to hire

Able to work harmoniously with all clients and staff members

Ability to transfer clients, lift, bend, stretch, sit, stand

What You Will Do:

Provide direct care to adult clients based on established care plan. Assist clients with activities of daily living skills including bathing, dressing, grooming, feeding, toileting, etc. Supervise, and/or assist clients in accordance with the person-centered plan, including personal care, recreation, and reinforcement of individual rehabilitation objectives.

Administer and document medications and treatments as prescribed by physicians and health care professionals. Chart each client’s daily documentation log including goals, completed tasks, and behavior before the end of your shift. Must be able to chart and use Electronic Medical Records software. Must be able to appropriately communicate and hand-off clients to the next shift team.

Moderate housekeeping/cleaning duties are assigned daily. Must follow Healthcare Facility guidelines regarding cleaning, disinfecting, and infection control protocols to ensure facility, client, and visitor areas remain in clean order at all times.

Prepare and/or serve meals according to a set menu and adherence to prescribed diets. Knowledge and ability to appropriately store leftovers and re-serve as appropriate is required. Initiate chores with clients and actively encourage the client when they lack initiation/motivation.

Schedule: Must be able to work Day or Afternoon or Midnight 8-12 hour shifts including holidays and weekends

Day Shift 7:00a-3:00p

Afternoon Shift varies 2:00p-11:00p

Night shift 10:00p-7:00a

Rotating weekends and alternate holidays are required

Salary: $16.00 - $17.00 per hour
