American Home Visiting Physican is seeking a compassionate and patient-focused Nurse Practitioner to join our team. As a Nurse Practitioner, you will provide comprehensive care, patient education, and care management to clients in their own homes, promoting their health and well-being.
Key Responsibilities:
American Home Visiting Physician provides face-to-face capacity to include patient residence and qualifying facilities.
Performing care and prescribed treatments to patient
Develop and document individualized care plans customized for each patient's unique needs
Requirements:
Nurse Practitioner License (Required)
BLS - Basic Life Support
