Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures . Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs . Qualifications

Required Bachelor's Degree or equivalent related field

Preferred Master's Degree or equivalent related field

LIC-Registered Nurse (RN) - STATEMI State of Michigan Excludes CV Administration, HM Patient Care Logistics, Health Comm - Department, GM-Community Education, Lifestyle Medicine, PD HDVCH Child Protection, PH Medical Affairs Admin, PH Pre-Service Utility Management required Or

LIC-License Practical Nursing (LPN) - STATEMI State of Michigan required Or

CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration required Or

CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants required Or

LIC-Master Social Worker (MSW-Master) - STATEMI State of Michigan or equivalent in mental health/behavioral health preferred Or
