Hayes Residence is seeking a dedicated and knowledgeable Dietitian to join our long-term care nursing home facility. The ideal candidate will possess a strong background in nutrition and dietetics, with a focus on patient care in various settings including acute care, geriatrics, and long-term care. The Dietitian will play a vital role in assessing patients' nutritional needs, developing personalized dietary plans, and providing health coaching to support weight loss and overall wellness. This position requires a thorough understanding of physiology and the ability to work collaboratively within a multidisciplinary team.
Responsibilities
Conduct comprehensive patient assessments to evaluate dietary needs and health conditions.
Develop, implement, and monitor individualized nutrition care plans based on patient assessments.
Provide education on nutrition-related topics to patients, families, and healthcare staff.
Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient care.
Offer health coaching to support patients in achieving their weight loss and health goals.
Stay updated on the latest research in nutrition and dietary practices to provide evidence-based recommendations.
Participate in quality improvement initiatives related to nutrition services within the facility.
Document all patient interactions and progress accurately in compliance with healthcare regulations.
Experience
Bachelor's degree in Dietetics or Nutrition from an accredited institution; Master’s degree preferred.
Registered Dietitian Nutritionist (RDN) credential is required.
Experience in acute care settings is highly desirable; familiarity with hospital systems is a plus.
Knowledge of geriatrics and long-term care practices is advantageous.
Proficiency in patient assessment techniques and developing tailored nutrition plans.
Strong understanding of physiology as it relates to nutrition and health outcomes.
Excellent communication skills for effective health coaching and education delivery.
