The Heights is a residential and IOP facility located in St Paul. We are seeking a motivated nursing candidate to help with the daily tasks associated with our facility. The position is full time, M-F ideally 7-4, 8-5, or 9-6. There is some flexibility on scheduling. Some of the tasks included are:
Count controlled substances with the on-coming and off-going staff.
Read shift report since the last shift worked to keep updated on changes.
Check appearance of any ill or injured client. Contact appropriate medical provider as needed. Provide treatment as ordered and document in medical record.
Administer routine and PRN medications and treatments as ordered. Document on the medication administration record/PRN sheet.
Obtain/record/review routine and as needed vital signs & weights. Notify Client provider of significant health changes.
Document pertinent information on clients' medical record according to policies and procedures.
Schedule appointments and send updated clinical documentation to the provider prior to the appointment. Update Procentive shared calendar with appointments, intakes, and transportation needs.
Order medications, sign in medications from Pharmacy.
Write referrals/lab slips.
Take telephone orders from physician/NP and transcribe orders.
Order lab studies; report any abnormal values requiring immediate medical attention to the Client Provider. Send copies of lab values to physicians.
Complete incident reports and medication error reports when needed.
Answer the phone by the third ring and transfer calls to appropriate staff member or voice mail.
Notify dietary department of dietary orders and changes.
Maintain safety of the clients and security of the building. Report/correct actual or potential hazards.
Maintain med room and Tech Office in neat and orderly manner for up-coming shift.
Attend In-services and staff meetings. Provide quarterly in service education for staff.
Monitor the health services related tasks completed by Chemical Technicians.
Respond appropriately to emergency situations.
Monthly MAR and Physician Orders review, setup, and obtain required signatures.
Health education to clients in group and staff in-services; under the supervision/delegation of the Director of Nursing.
File health paperwork and Procentive faxes into respective client charts; forward necessary paperwork to appropriate staff.
Maintain client health charts.
Any other duties as assigned by the Clinical Director.
Assist visitors/family/friends/physicians/lab/transportation personnel as needed.
Document physician, NP, psychiatrist orders after visits have been made.
Monitor level of med room supplies and notify Human Resources when an order is needed.
Admit/discharge residents according to health services policies and procedures.
