RN/LPN Private Duty/Hourly Nursing Position

Position Overview:

Willa Healthcare is seeking dedicated, compassionate, and experienced Registered Nurses (RN) or Licensed Practical Nurses (LPN) to join our Private Duty Nursing Team in a full-time or Part-time capacity. The Full-Time or Part-time Home Health Nurse provides skilled one-on-one nursing care to clients in their homes, ensuring safety, comfort, and continuity of care under the supervision of the Director of Nursing.

Primary Duties and Responsibilities:

Provide skilled nursing care to clients according to physician orders and individualized care plans.

Conduct initial and ongoing comprehensive nursing assessments to monitor client status and update care plans as needed.

Administer medications, treatments, and skilled interventions (e.g., wound care, trach care, diabetic management, etc.).

Maintain accurate documentation and electronic charting standards.

Observe and report client changes promptly to the physician and Director of Nursing.

Educate clients and families on medication management, chronic condition care, and safety practices.

Coordinate with multidisciplinary team members to ensure seamless care delivery.

Adhere to compliance requirements.

Participate in staff meetings, ongoing education, and agency quality improvement initiatives.

Demonstrate professionalism, accountability, and respect in all interactions with clients, families, and colleagues.

Minimum Qualifications:

Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing with the Minnesota Board of Nursing.

Minimum 1 year of clinical nursing experience (home health, long-term care, or hospital setting preferred).

Current CPR/BLS certification required.

Valid driver’s license, reliable transportation, and proof of auto insurance.

Excellent assessment, problem-solving, and time management skills.

Strong communication and documentation abilities.

Must pass a background study, employment health screening, and reference checks.

Proficiency with electronic health record (EHR) systems preferred.

Preferred Qualifications:

Experience in Private Duty Nursing (PDN) or skilled home health care.

Competency in specialized areas such as trach care, ventilator management, or enteral feeding (training available).

Bilingual ability (English/Spanish, Somali, or Hmong, e.t.c) is an asset but not required

Work Schedule:

Full-Time: 32–40 hours per week, or part-time, flexible scheduling based on client needs. Shifts available: day, evening, night, and weekend rotations. Schedules are typically established 4–6 weeks in advance to support work-life balance. Overtime opportunities may be available base on client’s needs.

Compensation & Benefits:

Competitive hourly rate: $30 – $40 per hour, based on licensure and experience.

Work Environment:

Care is provided in client homes across the Twin Cities Metro Area (Counties: Dakota, Hennepin, Scott, Rice, Carver, Ramsey, Goodhue, Washington, Anoka). Must comply with all infection control, safety, and clinical care protocols established by Willa Healthcare. Nurse must maintain confidentiality, professionalism, and client-centered care at all times.
