Hospice Cer tified home health nursing aide\'s (CHHA) main role is to provide direct patient care in the home or setting where the patient refers to as home. Hospice Certified home health aides perform basic duties such as bathing, assistance with activities of daily living, safety and vital signs. The CHHA must demonstrate competency with all core skills as required by the Certified Nursing Assistant licensure. Demonstrates effective verbal and written communication skills. Understands basic knowledge of ethical issues and is able to provide care to terminally ill patients. Provides caring holistic person-centered care. Demonstrates knowledge of basic infection control practices, such as hand washing technique. The CHHA is under supervision of the Hospice clinical coordinator/supervisor and/or director. Ability to communicate with the Hospice RN case manager for concerns that may arise in the home while performing patient care. On a daily basis communicate with the CHHA scheduler. The CHHA follows an individualized plan of care that is developed by the RN case manager. The CHHA will demonstrate an understanding of the plan of care prior to the first visit with patient. Maintains HIPPA compliance. Has a general understanding of regulatory compliance to assure the provision of safe, quality care.

Demonstrates knowledge of the nursing process as a standard, incorporating elements of holistic person-centered care through the collection, verification and organization of data about a patient’s health status, planning for care, implementation of the plan and evaluation. Provides direct patient care in the home, general inpatient setting or setting where the patient refers to as home. The RN is under the direction of the Director or designee. The RN performs detailed and accurate patient assessments utilizing a comprehensive assessment. Completes documentation in a timely and accurate manner according to hospice policy and regulatory requirements. Facilitates patient hand-offs and provides assistance to move the patient through life transitions. Understands the principles of case management to ensure effective coordination of care and utilization of community resources. Effectively manages a group of hospice patients. Works collaboratively with other members of the interdisciplinary team (IDT) physicians, families and the community emphasizing a teamwork environment. Ability to implement quality care standards for patients served. Coordinates provision of patient care for assigned patients, maintains competency levels annually and attends continuing education as applicable. Delegates appropriate tasks as needed. Works with the Director and/or clinical coordinator/or manager to assure patient visits are being provided as outlined in the plan of care.

Is able to intake patient, verification and recording of pertinent patient health history, while providing excellent patient care. Able to prioritize workload, respond to patient calls and messages in a timely manner. Demonstrates knowledge of the nursing process as a standard in accordance with licensure, incorporating elements of holistic family centered care.
