Key Responsibilities:
Provides general clerical support to the HCH Director and Team as appropriate.
Provides assistance in distributing and coordinating reports to HCH Team, Community Support Specialists (CSS), and community providers when needed.
May serve as a CyberAccess Practice Administrator responsible for maintaining user accounts and monitoring agency usage and prescriber reports, and promotes the use of Direct Inform.
Provides technical assistance to HCH Team and CSS’s on use of CareManager and CyberAccess.
Assists with tracking that all required client screenings and reporting measures are complete and submitted as appropriate.
Attends state required meetings and trainings.
Maintains a monthly log of HCH activities and may complete the monthly HCH Team Log.
Prepares reports to be submitted to HCH Director and Assistant Director before submitting to DMH every month.
Is responsive to all Quality Assurance procedures of Burrell, cooperates with utilization review procedures, and follows established procedures, rules and regulations of the Clinical Records Department.
Performs other special functions as may from time to time, be outlined and attached hereto.
Education and/or Experience Qualifications:
HS Diploma or GED required.
Additional Qualifications:
Knowledge of chronic health conditions in both youth and adult populations
Excellent written and verbal communication skills
Knowledge of mental health conditions in youth and adult populations
Ability to make decisions based on critical thinking
