Experience in working with individuals with mental illness and substance use disorders a plus
An ideal candidate will perform the following:
The provider will take blood pressure, weight, input vital statistics and medications into the patients chart.
The provider will additionally assist the Nurse Practitioners with insuring that documentation is appropriately conducted and stored in the Electronic Medical Records system. The provider will assist the Nurse Practitioners in completing refill requests, contacting primary health care providers, sending and obtaining medical records.
The provider will document and provide services including appropriate case management and supervised medical assessments, monitoring, advocacy, and treatment planning.
The provider will assist with inventory of medications with direct supervision of our nurse practitioners and/ or the RN.
The provider will provide assistance as necessary for the medication coaching and delivery program. The provider will work with participants to increase independent medication routine in their home settings.
