Mountain Home seeks a Case Manager (CM) to help improve outcomes, experiences, and value for the families served. The CM provides case management to young mothers, adolescents, and children, linking them to needed medical, social, educational, vocational, and recreational services and providing advocacy to support and increase their independence in the community.The ideal candidate is familiar with social services in the Missoula community, has knowledge of mental illness and treatment, has two or more years of experience providing case management or similar services, and has or is working toward a bachelor’s degree in social work, human services, or a related field.
Status: Non-Exempt, Non-Shift, Full-time, 36 hours/week
Pay Range: $19.50 - $24.50, based on years of experience, education, and training

Schedule: Monday - Friday with some evenings (no later than 8:00pm) required

Travel: This position requires frequent local travel, approximately 30% of weekly time using company vehicles
