Children's Case Manager

Mountain Home seeks a part-time Children’s Case Manager (CCM) to help improve outcomes, experiences, and value for the families served. The CCM provides intensive case management to children between the ages of 0 and 18 through the implementation of individual treatment plans, case coordination, and advocacy. The Children’s Case Manager links children to needed medical, social, educational, and recreational services.

The ideal candidate is familiar with children’s social services in the Missoula community, has knowledge of mental illness and treatment, has two or more years of experience providing case management or similar services to youth, and has or is working toward a bachelor’s degree in social work, human services, or a related field.

Status: Non-Exempt, Non-Shift, Full-time, 20 hours/week

Pay Range: $19.50 - $24.50, based on years of experience, education, and training

Schedule: Primarily afternoons 12:00pm - 6:00pm with the expectation of some mornings and occasionally evenings no later than 8:00pm as dictated by client need.

Location: 80% of work will be performed in the community, client homes, or office

Travel: This position requires frequent local travel, approximately 30% of weekly time using company vehicles
