DescriptionThis position is grant-funded which may expire on or before June 30, 2025. Examples of Duties

Operates independently to coordinate the implementation of program functions, student intake and assessments, marketing, outreach, and family engagement

Assists in developing and presenting workshops and classroom instruction to students and parents that focus on proven strategies leading to successful transition

Administers required assessments and evaluations

Coaches students in academic or vocational concepts

Completes Individual Transition Plans and ensures goals created within the plan are addressed

Identifies community resources and makes referrals for students and their families

Monitors and maintains student records to track goals and to ensure accurate reporting according to program regulations

Prepares and submits requests, information, and reports related to the Center for Transition Success

Responds to the potential participant and other program inquiries

Represents the Center for Transition Success at internal and external events

Serves as backup for other Workforce Development members as assigned

Performs related duties as assigned

Typical QualificationsEducation:

Bachelor’s degree in special education or a related field

Experience:

3-5 years of experience working with individuals with disabilities and their families

Execution of an Individualized Education Program (IEP) or 504 Plan

Competencies and Skills:

Excellent communication, interpersonal, and organizational skills

Knowledge of special education rules and regulations and the transition process

Knowledge of the Americans with Disabilities Act and other federal and state regulations related to individuals with disabilities

Microsoft Office Applications (Word, Excel, PowerPoint)
