JEWISH FAMILY AND CHILDREN’S SERVICE OF SOUTHERN NEW JERSEYJOB DESCRIPTION: Geriatric Social Worker (Full-time)Reports to: Director of Senior Services Department Primary Responsibilities: Provide case management and social work services to maintain and improve functioning of senior clients and their families in accordance with Senior Services policies, procedures, and goals. Provide outreach, education, and networking opportunities with community resources to enhance client/family care.Specific Responsibilities:
Conduct individual multifunctional assessments and ongoing client care coordination, including, depression and mini-mental screenings to augment existing services or act as a guide in determining or changing level of service
Provide ongoing case management services and patient advocacy to Senior Services clients to maintain their independence in the community
Assess clients’ financial situation and make appropriate referrals to ensure entitlements
Develop with client and family, wherever possible, a realistic plan, for both long-term and short-term, including viable options
Prepare accurate and timely records, including clinical and progress notes
Share information regularly with care team and supervisor
Participate in conferences and peer supervision with members of staff to share relevant information or resources
Provide consultations with clients and/or family members and refer to proper community resources
Provide intake coverage for the Department
Assist, when assigned, administrative and management tasks of programs
Participate in in-service trainings
Participate in Senior Services and agency wide staff meetings
Serve as a member on agency committees, including PQI and QAUR, as needed
Represent the agency at community programs and outreach events
Train and interface with volunteers and community members for specific programs
Facilitate any new agency workshops/programs or support groups, as needed
Qualifications:
Masters in Social Work (MSW) or comparable degree required, License preferred
Three years’ experience in a health or social service agency, hospital, or institutional setting
Knowledge of social work methods and community resources
Experience working with elderly preferred
Knowledge of Jewish culture
