Adult Medical Day Care in Somerset has a part-time Social Worker opening position with adult medical day care experience. The Social Worker shall be licensed or certified by the New Jersey State Board of Social Work Examiners.
At least social services to be provided as follows: interviewing the participant and caregiver to obtain a social assessment and evaluation of needs and problems, assisting with obtaining concrete services such as, food, housing, clothing etc., providing family outreach, documenting assessments, evaluations and interventions, discharge planning shall include linking the participant to necessary community services.
