RN – Client Liaison (20-30hrs)
Job description
Position Overview:
We are seeking a compassionate and skilled Registered Nurse to join our Home Care
team. The successful candidate will be able to multi task to assist our Agency in
providing quality Home Care to our Home Care clients. We take pride in our white glove
services and are seeking a RN Client Liaison who is compassionate yet has a strong
understanding of home care standards. The role requires applicant to have a current
New Jersey Registered Nurse license. This position is office based with required client
field visits and back up on-call responsibilities. Position offers flexible hours.
Duties:
Client home visits as necessary to complete assessments, POC development and any
other required
documentation
Collaborate with the healthcare team to develop individualized care plans, and contact
client & doctor as necessary
Monitor and maintain client records, identifying any potential health concerns
Orient new field staff to policies and procedures
Orient and supervise aide staff in the field
Teach in-services as needed
Back up on-call to DON
Field staff supervision and orientation of tasks/duties
Orientation to agency policies and in-service training for field staff
Ensure compliance with occupational health and safety standards.
Maintain confidentiality and adhere to all relevant healthcare laws and regulations.
Skills/Requirements
- Commitment to delivering exceptional care.
- Proficiency in client record documentation and attention to details.
- Bilingual a plus, but not required
- Minimum of 1 years RN experience
- Ability to work efficiently in the field and an office setting
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills, with a focus on patient-centered care.
- Knowledge of occupational health and safety standards in the home care industry
Job Types: Full-time, Part-time
Pay: $38.00 - $45.00 per hour
Expected hours: 20 – 30 per week
