CareLink Community Support Services has a current opening for a Program Coordinator at our New Jersey A+ Program located in Clementon, NJ. Our rehabilitation services and professional staff provide the supports that people with serious mental illness, physical disabilities and other developmental challenges need to be active, productive members of their communities.

This position is responsible for the oversight of Residential Counselors in the absence of the Program Supervisor. Provides oversight to the shift and ensures administrative supervision of the residential staff and operations of the program.

Available Shifts:

1st Shift: (40 Hours) Monday through Friday, 8 AM - 4 PM

Salary Range:

$16.50/hour

Qualifications:

1. Educational Requirement:

Bachelors degree from accredited institution in mental health, health, or related human services field or

Licensed Registered Nurse or

Two years of college plus two years of related work experience or

Licensed Practical Nurse and two years of related work experience or

High school diploma/GED plus four years of related work and life experience

2. One year of experience in a residential mental health setting.

3. Proficiency with computers and experience with an Electronic Health Records system (EHR) is preferred.

4. Previous supervisory experience is preferred.

5. RPRP/CPRP certification preferred.

6. Knowledge of basic principles and methods of rehabilitation required.

7. Motivation, interest and ability to work empathetically with people with disabilities.

8. Ability to work independently required.

9. Must be able to write and speak English in an understandable manner.

10. Ability to effectively manage multiple tasks required.

11. Ability to take direction.

12. Ability to prepare meals, perform general housekeeping and maintenance duties required.

13. Ability to make sound judgments regarding the health/welfare of participant required.

14. Valid state drivers license required.

15. Ability and willingness to drive agency van required.

16. Ability to handle and maintain confidential information required.

17. Lifting is required. Ability to lift at least a minimum of fifteen (15) pounds. Ability to physically assist consumers with specific needs in relation to lifting.

18. Some knowledge of psychotropic medications required.

19. Ability to work as a leader and provide training to staff required.

Essential Functions:

1. Provide leadership and administrative supervision to all staff by providing instruction, leadership, example, and suggestions as appropriate.

2. Ensure coverage for each shift through various means: asking on-duty employees to remain, calling staff roster, etc.

3. Provide adequate information exchange at end of shift with on-coming supervisor or staff.

4. Plan shift operations of the site including shift assignments, reporting all problems to the Program Supervisor.

5. Assist in providing an orientation for new employees in the agency and facility philosophy, policies and procedures.

6. Participate in performance reviews for employee introductory, annual and interim evaluations.

7. Participate in development and training of staff to improve quality of services provided to participants.

8. Participate in and assume leadership role during staff meetings and training programs.

9. Provide coverage at site when needed.

10. Report all concerns regarding staff to Program Supervisor.

11. Attend and participate in supervision.

12. Coordinate referral and intake process as directed by Program Supervisor. Complete only non-clinical and non-medical portions of the comprehensive intake assessment.

13. Conduct initial and ongoing assessments of consumers strengths, interests, and areas needing assistance.

14. Assist in the development, implementation, and monitoring of goal plans and treatment interventions.

15. Coordinate the orientation of new consumers, take inventory of all personal items upon admission, and conduct introductions to staff and participants.

16. Coordination of all consumer appointments.

17. Assist participants in planning, coordination, and implementation of daily, evening, and weekend social and recreational activities.

18. Participate in the development of individual service plans designed to improve the social and self-care skills of program clients.

19. Attend monthly case management meetings when appropriate.

20. Coordinate site meetings.

21. Review all documentation generated during the shift to ensure that work is accurate, complete and finished in a timely manner.

22. Monitor medication of all participants as prescribed and document as required. Report all medication errors to Program supervisor.

23. Prepare consumers progress reports and other reports as required.

24. Coordinate individual services with outside agencies and individuals as appropriate.

25. Execute professional daily progress notes, log book entries and other related paperwork as required.

26. Assist with payroll processing as necessary.

27. Participate in on-call rotation as necessary.

28. Respond appropriately to emergency situations and provide crisis intervention services.

29. Seek clinical support from the Program Supervisor during clinical emergencies.

30. Transport clients as necessary/appropriate.

31. Perform job within framework of agency, site and professional standards.

32. Participate in staff meetings and training programs.

33. Participate in supervision on an on-going basis.

34. Attend and participate in agency events.

35. Adhere to agency/site policies and procedures.

36. Project a positive attitude and image to participants, family members and outside contacts.

37. Use appropriate communication channels.

38. Participate in achievement of agency goals and mission.

39. Display motivation and interest in working creatively with people with disabilities.

40. Demonstrate the ability to make sound judgments regarding the welfare of residents.

41. Display ability to take direction.

42. Possess knowledge of basic principles and methods of rehabilitation.

43. Follow expected dress code.

44. Adhere to supervisor's requests.

45. Cooperate with other staff and exhibit positive attitude toward position and agency mission.

46. Show initiative and demonstrate good judgment.

47. Participate in shifts as scheduled by supervisor.
