APH, Inc. is seeking a new team member. Are you a committed and dedicated Certified Peer Support Worker? Our CPSW will provide case management services specific to the needs identified, assist clients in achieving goals as outlined in action plans, and promote skills for coping with and managing symptoms; facilitate workshops to empower, equip, and encourage clients’ activities, whether promoting and emphasizing recovery and rehabilitative services to prepare clients for successful reentry into their families and communities. Other duties are as follows:
Obtain relevant information from clients to create recovery, retention & relapse prevention plans
Effectively communicate with staff, volunteers, and providers as needed
Teach and role model the value of their individual recovery experience
Facilitate/co-facilitate groups
Maintain contact with community agencies and resources essential for successful case management
Document the case management process, adhere to HIPAA confidentiality mandates, and follow and abide by the organization’s ethical standards of practice
Assist clients with supportive services for housing, ISD, Medicaid/Medicare, SSA, employment, and any other needs that may arise
Implement motivational interviewing strategies to empower clients.
Engage with clients to help them recognize goals related to achieving Independence.
Assist clients in identifying immediate needs and breaking them down into small, attainable steps
Attend staff meetings
May be required to work weekends and/or evenings for special events or outings with women
May be needed in other capacities of the organization – Must be a team player
