Patient Assessment: Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results.
Diagnosis and Treatment: Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed.
Preventive Care: Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling.
Health Education : Educate patients and their families about health conditions, treatment options, medications, and self-care techniques.
Care Coordination: Collaborate with the Medical Director, Operations Manager, Care Navigators, and Medical Assistants to streamline patient care.
Telehealth: Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform.
Record Keeping: Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards.
Prescription Management: Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed.
Compliance: Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines.
Community Health: Support CHI initiatives aimed at reducing healthcare disparities in underserved communities.
Follow-ups: Ensure that patients receive appropriate follow-up care, either personally or through coordinated care teams.
Requirements:
Education: Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited nurse practitioner program.
Licensure: Possess a valid and unrestricted nurse practitioner license in the State of New York,
Certification: Be certified as a nurse practitioner by a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
Clinical Skills: Demonstrate proficiency in conducting physical assessments, diagnosing medical conditions, and developing treatment plans.
Communication Skills: Have excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare team members.
Bilingual: Fluency in Chinese (Mandarin or Cantonese) is required to effectively communicate with diverse patient populations.
