Nurse Practitioner (NP)Location: Buffalo, NYEmployment Type: Full-TimeCompensation: Range: $118,000 annuallyWe are seeking an experienced Nurse Practitioner to join our team, focusing on member engagement and care management for high-risk populations, with an emphasis on black and brown communities. The role involves assessing healthcare needs, developing patient action plans, managing care transitions, and coordinating care for patients with complex, chronic medical and mental health conditions.Primary Responsibilities:
Develop and use care pathway templates by chronic condition.
Deploy remote patient monitoring and patient self-reporting tools.
Conduct in-home or tele-health assessments and respond in real-time to patient alerts.
Provide care coordination, patient navigation, chronic disease management, and interdisciplinary collaboration.
Engage patients in managing their health, medications, treatments, and follow-up appointments.
Background and Experience:Bilingual in Spanish or Burmese preferred.Background in OB GYN, labor and delivery in a hospital setting.3-5 years of experience as a WHNP, FNP, or CNM.Requirements:
Certified and registered as a Women’s Health Nurse Practitioner (WHNP), Family Nurse Practitioner (FNP), or Certified Nurse Midwife (CNM) by the New York State Education Department (NYSED).
Currently registered and licensed as a registered professional nurse (RN) in New York State.
Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
Completed pharmacology coursework acceptable to NYSED.
Applied for nurse practitioner certification with NYSED.
