What you'll do The Program Manager of Value Based Contracting for Population Health will oversee value-based incentive contracts. Works as part of the population health leadership team to ensure the success of the programs and interventions that guide the delivery of high quality, value driven, healthcare to improve population health outcomes across the Bassett Healthcare Network (BHN). The Program Manager will provide operational leadership for the interior design and review process of the programs to ensure efficiency, effectiveness and be operationally linked to financial success in value-based arrangements. Provides leadership and explores strategic opportunities through relationships with payors. Responsible for value-based contracting for population health. Works directly with the Population Health leadership and payer sources to develop and maintain incentive contracts Develops and maintains financial tracking of all incentive plans. Collaborates with finance to ensure payer contracting and incentive allocation Coordinates with leadership and other team members to support the obligations of incentive plans. Identifies risks pertaining to value-based incentive programs and provides solutions Has subject matter expertise and maintains knowledge of CMS and other Quality Payment program Works with Population Health Initiatives and processes with practitioners while facilitating universal standards of care Develops strategies and implements objectives to ensure efficient and effective program performance which correlates with the institutional annual operating plan Monitors and reports program performance related to established objectives Communicates with the Population Health Director regarding operational objectives, planning, daily operations and regulatory requirements Handles initiative expense reimbursement/incentives for practitioners and staff Participates in multidisciplinary work groups and teams to facilitate efficient, effective operations and high-quality program outcomes Monitors and assesses performance goals, providing feedback and direction to division staff members in coordination with departmental directors and site managers Ensures compliance with all regulatory agency requirements in conjunction with the departmental directors and site managers Participates in multidisciplinary committees, work groups and teams to facilitate efficient and effective operations and effect safe patient care where applicable Works collaboratively with other leaders from partner organizations to coordinate services, integrate operations to the highest level possible and insure seamless delivery of care and services to patients Establishes positive working relationships with all levels of management, physicians and staff in partnership Maintains current knowledge of trends in the insurance arena and associated programmatic initiatives through reading and ongoing educational opportunities –makes recommendations for operational improvements accordingly Tracks and reports performance improvement activities as appropriate and/or required by institutional guidelines What we need from you Education: 4 Year/Bachelor's Degree Nursing degree, required related Associates Degree with additional 3 years direct experience may be considered in lieu of Bachelor's education Master's Degree or RN, preferred Experience: 3 years Healthcare contract management experience, required 5 years Healthcare contract management experience with value-based contracts, required Skills: Accountability Communication Community Relations Computer Skills Interacting with People Leadership Manage People Reading/Writing/Comprehension Reliability Time Management Team Building Physical Requirements: Sedentary Work - Exerting up to 10 lbs of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Pay Range Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $91,367.57/Yr. Pay Range Maximum: USD $137,051.36/Yr.
