The Case Manager is responsible for supporting the psychosocial needs of residents. Duties include conducting initial evaluations and 90-day assessments, ensuring residents health records are maintained and updated, and performing quality assurance monitoring. You will be responsible for prescreening residents and assisting with their ongoing care needs, collaborating with other team members to promote the well-being and quality of life of the residents.
