The Registered Nurse ensures the health and physical well-being of the consumers and will;
Participate as appropriate in the development, review and update of an Individual Program Plan as part of the Interdisciplinary Team process
Participate in pre-admission screening and evaluation of ICF-referred Consumers
Develop an initial nursing assessment within 24 hours of placement at a facility
Develop a Nursing Service Plan based on a Nursing Assessment, Physical Exam Report, Evaluation of Consumers Ability to Self Medicate and ITT input, within 30 days of placement, and annually thereafter
Review and updates Nursing Service Plans semi-annually and as needed based on needs derived from completed nursing evaluations and quarterly nursing physical assessments
Ensure that nursing care, as prescribed by the physician or as identified by individual needs, is carried out
Participate in the development of discharge plans
Make referrals to appropriate community resources based on the completed medical portion of the consumer’s Community Service Plan
Assesse consumer’s health condition as needed, and determines the need for referral to physician or emergency room based on observation and/or staff provided information
Ensure control of communicable diseases and infections
Supervise medication administration periodically, ensuring adherence with OPWDD regulations and agency policy
Report all injuries to management immediately to ensure appropriate incident reporting procedures are followed
Complete quarterly Quality Improvement Reports and ensure resolution to any outstanding issues
Review and follow up on Quality Improvement Reports completed by the Medical Assistant
Complete semi-annual and annual nursing ITT report
Participate in annual and semi-annual interdisciplinary team meeting along with any special meetings within or outside the residence
Participate in the monthly review and reordering of consumers’ medication together with the primary psychiatrist and facility psychologist
Ensures proper storage of all medication and medical supplies, and the proper disposal of the same (in accordance with OPWDD regulations and/or agency policy)
Review and monitor, for the purpose of health maintenance bowel movement records, vital signs, seizure activity and records, menstrual records, and tardive dyskenesia records on an ongoing basis. Reports irregularities or abnormalities to the consumer’s primary physician as needed
Ensure ongoing health care services are provided as needed and medical appointments are scheduled in a timely manner
Supervise, assists and trains a qualified program specialist to become a Nursing Assistant
Supervise, assists and trains Licensed Practical Nurses to include a review and signing of all written reports for accuracy prior to being presented to ITT
Registered Nurse licensed to practice in New York State
Specialized training in IDD and/or one year experience in community health care services
AMAP trainer
CPR and First Aid Trained
