Under the direct supervision of the Director of Behavior Health Service, we seek a full-time licensed clinical social worker to provide mental health therapy for patients in its Outpatient Behavioral Health Program.
Main Responsibilities:
Ability to work across the age spectrum and working with families.
Performs Mental Health Intakes for patients to ascertain their diagnoses, determine an appropriate level of care, and establish a treatment plan
Provides ongoing individual and/or group therapy, maintaining ultimate responsibility for directing the course of the patient's mental health care and treatment plan within the scope of their expertise
Provides crisis intervention to diffuse conflicts among patients
Ensures a welcoming, unbiased, and inclusive environment for all who seek care
Completes medical forms and documentation related to patient care, including any necessary to facilitate transfer and if indicated, referrals to outside programs or psychiatric clinics
Encourages health promotion, disease prevention, and health maintenance
Provides patient advocacy, as appropriate for the site
Completes documentation, including diagnosis, treatment, and billing, thoroughly, accurately, and in a timely fashion
Makes and/or tracks all referrals to other mental health agencies and providers
Works collaboratively with specialists for consultation and referral purposes, collaborates with psychiatric providers and medical providers within and across the clinic as needed
Maintains confidential case files, including documenting requests for confidential information
Follows the clinical chart review policies and procedures
May provide workshops for families on communication and other family-related issues
Coordinates emergency responses, including a team to minimize the patient's danger to self and others, direct discussion with EMS/FDNY regarding the patient's condition, referral to psychiatric or medical emergency departments, and follow-up while the patient is in the emergency department or upon release
Participates in case-conference and case management activities
Utilizes supervision effectively to improve patient care; engages in critical reflection to develop personally and professionally
Qualifications and Requirements:
Minimum of two years of clinical experience.
Fluency in English and Spanish is preferred.
Possess a valid, unrestricted New York State Licensed Clinical Social Worker or Clinical Psychologist license.
Prior experience with substance abuse issues preferred.
Substantive knowledge of EMR systems, Athena is preferred.
Salary:
$80,000/annually
