Promotes Person-Centered planning for people receiving services and their family members/guardians. Collaborates with the entire residential team including the Residential Nurse, Residential Director, Residential Manager, Assistant Residential Manager, DSPs, Behavior Intervention Specialist, clinicians, community agencies, and others involved in medical/nursing care providers as appropriate. Provides oversight of the residents’ medical care as designated by the Residential Nurse and provides guidance to direct care staff regarding the residents’ medical needs. Ensures completion of required duties for each shift.ESSENTIAL FUNCTIONS/RESPONSIBILITIES

Coordinates referrals to community-based medical specialty providers

Assists with scheduling and attending appointments and procedures with persons served as needed . Uses Outlook to maintain the schedule. Ensures transportation and proper staffing ratio for the appointment is scheduled

Coordinates incoming and outgoing provider correspondence

Completes necessary medical and insurance forms as required and correspondence as needed

Maintains Medication Administration Certification and administers medications

Assists in delegated nursing tasks as directed by RN (e.g. PONS monitoring, tracking, audits, etc).

Responsible for the organization and cleaning of the medication room.

Ensures sufficient and accurate stock of medications at all times

Communicates with clinical staff to ensure follow-up to medical appointments in their clinical area.

Liaison with the pharmacy.

Checks MARS for errors and completes necessary paperwork including use of PRN medication and ensuring documentation is complete.

Completes all required documentation in eVero and QuickMar

Communicates with the person and team regarding medical appointments and outcomes.

Purges paperwork on an annual basis in conjunction with site nurse.

Makes notifications to day programs related to individual health needs and concerns.

Attends support team/clinical meetings as medical liaison.

Maintains a safe orderly environment conducive to the health, safety and wellbeing of individuals and employees.

Ensures infection control practices are followed

Provide supervision and training of medical needs to direct care staff on designated shifts as needed.

Safely transports individuals to and from activities and appointments.

Completes direct care tasks as needed/ directed by management

Complies with all agency policies and procedures.

Other duties as assigned.

QualificationsEducation: High school diploma or GEDCredentials: NYS Driver’s License that meets agency policy. CPR/FA, AMAP, SCIP certification required.Experience: At least 2 year(s) of experience

Customer relations, hospital experience or satisfactory completion of related medical practice training program. Experience with coding procedures is desired. SKILLS: Minimum: Must demonstrate the ability to communicate clearly and pleasantly to staff, physicians, patients and visitors. Able to perform under pressure, (relate to patients, hospital staff, physicians and visitors) Salary Range: $18.03-$19.18
