Job Details Work Schedule - Tuesday - Saturday Responsibilities: Perform discharge- planning assessment, when indicated, within the required timeframe. Foster a collaborative relationship with the medical staff and other members of the health care team to provide quality patient care and reduce LOS. Calls/ conversations with physicians regarding discharge planning needs. Bedside rounding with physician as often as possible. Collaborates with members of the health care team to achieve a cohesive and comprehensive discharge plan for patients. Develop discharge plans that are responsive to patient/ family concerns, health care status, available resources, age-specific problems and diagnosis specific issues. Setting up discharge plan based on the assessment- dual plan if necessary. Finalize plan prior to end of acute illness and setting up day of potential discharge. This will require taking the lead and informing team/ MD/ MLP of time of discharge/ pick-up. Provide appropriate referrals for patient/family to various resources. Perform third party payer reviews as requested within the appropriate timeframe and document it. Qualifications: Graduate of an accredited school of social work; Graduate of Master's degree in social work required. Two to three years' experience in health care related discipline (hospital, agency, etc.) and/ or previous discharge planning experience and/or community health care experience preferred. Must be extremely motivated, organized, flexible, and detail oriented; Excellent interpersonal skills (oral and written). Ability to respond to short deadlines. Effective problem solving abilities. Must have as S.W. license in New York State; Screen certification preferred. LMSW by the State of New York required. LCSW by the State of New York preferred. Salary Range USD $36.50 - USD $51.00 /Yr. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
