Req Number 142169Job DescriptionFacilitates patient admission, discharge, intra-hospital and inter-facility transfer processes through clinical triage. Acts as the primary liaison between physicians, hospitals and other care providers who seek to refer, schedule or transfer patients into the organization. Provides clinical expertise, close medical communication and quality assessment for the successful referral and transfer of patients to the facility.Job Responsibility
Promotes optimum utilization of hospital beds; conducts rounds and meets with staff regarding bed availability and current census.
Assigns patients to available beds to avoid delays in patient arrival to units.
Communicates with the surgery department, emergency department, nursing units, physician offices and outlying facilities to ensure appropriate placement of patients in a timely manner.
Assists with and ensures admission paperwork is complete, and ensures hand-off of communication to accepting nurse.
Ensures beds are assigned based on level of care and patient aggregation; addresses and resolves issues arising from increased census and decreased bed availability.
Promotes patient, family and physician satisfaction regarding bed assignments; addresses and resolves conflict regarding dissatisfaction with bed assignments.
Address any conflicts or issues arising from dissatisfaction with bed assignments.
Ensures patient information and data entry in patient's chart is accurate, including but not limited to insurance information, clinical documentation, transportation requests, and patient demographics.
Evaluates, processes, tracks, documents, and coordinates patient transfers for the successful referral and transfer of patients to the facility; may conduct recorded three-way calls between referring physician and facility physician.

Job Qualification
Bachelor's Degree in Nursing (BSN) required.
Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
2-5 years of technical experience and 0-2 years of leadership / management experience, required.
