Req Number 130277Job DescriptionInterprets, compounds and dispenses medications and other pharmaceutical supplies, using standard physical and chemical procedures to fill written prescriptions issued by physicians, and other qualified prescribers. Enters drug orders into computer profile.Job Responsibility
Reads and interprets physician medication orders.
Maintains patient medication profile including, but not limited to,orders, changes discontinuation and drug allergies.
Monitors and documents patient drug therapy including, but not limit to dose, timing, drug interactions or contraindications.
Notifies and consults with the physician (and nurse when appropriate) on drug dosage, drug interactions or contraindications, medication errors or omissions, non-formulary requests, etc and documents this consultation when appropriate.
Functions as the drug information specialist to professional staff on drug therapy.
Conducts and participates in educational activities for health professions, staff, students of various health care professions and patients.
Formulates and prepares pharmaceutical dosage forms, as needed.
Prepares I.V sterile drug product solutions, including chemotherapeutic agents using proper aseptic technique.
Labels medication products.
Discards refuse in strict accordance with hospital policies and procedures.
Participates in multidisciplinary patient care rounds providing drug information and ensuring optimal drug therapy.
Maintains statistics of clinical and distributive tasks.
Conducts reviews of medication storage areas.
Performs related duties as required.

Frequent physical travel through pharmacy and patient care areas along with the ability to stand, sit and climb stairs
Occasional lifting up to 30lbs.
Job Qualification
High School Diploma or equivalent, required. Knowledge of Pharmacy Science and Practice as normally acquired through completion of a five (5) year Bachelor's Degree in Pharmacy.
Must possess current license to practice Pharmacy in New York State.
Minimum of one (1) year related experience to ensure familiarity with hospital pharmacy practice, preferred.
