HOURS: Full-Time 40 hours per week Sunday OFF Monday OFF Tuesday 10:00 am - 6:00 pm Wednesday 10:00 am - 6:00 pm Thursday 10:00 am - 6:00 pm Friday 8:00 am - 4:00 pm Saturday 8:00 am - 4:00 pm Hours May Vary on Department Needs Scheduled hours subject to change based on department/participant needs. Main Function: Provides needed ongoing supervision and support for persons who receive services in the community residence program. Duties:

Provides assessment, teaching and evaluation of various skills of daily living that will enhance the independence and community integration of persons who receive program services.

Develops supportive relationships with persons who receive program services in the framework of a team-oriented approach that will foster an atmosphere in which skill teaching and role modeling can occur.

Assists the Program Manager in the tasks of providing case management services on behalf of persons who receive program services.

Documents plans, assessments and services provided in a manner consistent with Agency expectations and government regulations.

Uses the appropriate vehicles of communication for contact with other staff members in both ordinary and unusual circumstances.

Responds to crisis situations using good judgment in accordance with Agency guidelines and programmatic considerations.

Maintains physical property of residential program in a manner that enhances the health and safety of persons who receive program services.

Provides support to the Program Manager and other staff members in the framework of a team-oriented approach.

Performs other related duties as requested.

Education & Experience: High School Diploma or GED. Conditions of Employment: Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving policy. Must have own transportation, must be able to travel and have a flexible schedule to accommodate program needs. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the New York State Office of Mental Health. Must participate in successful database checks through the HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals. Must participate in successful database checks through the State Central Registry of Child Abuse and Maltreatment. Occasionally on-call at home in case of program needs or emergencies. Necessary to be able to hear clearly and have visual acuity. Must possess good verbal communication skills. Must be able to climb stairs, and to walk, stand, stoop, reach, and have good hand, foot and eye coordination. Must have manual dexterity and be able to read and write. Must be able to lift and carry at least 50 lbs. May need to be outside in extremely cold, hot or wet weather. Must work with others 75% of the time, must be able to sit in meetings for at least 1 1/2 hours at a time. Necessary to have experience in supporting parents in the parenting role, including parent training and referrals to appropriate community resources.
