Comprised of a network of agencies across Albany, Rensselaer and Schenectady Counties, the CRHC Health Home program serves some of the most needy and vulnerable through care coordination. The ideal candidate for the Patient Safety and Quality Improvement Specialist will be someone who exhibits compassion and understanding regarding the work CRHC does to support a complex and vulnerable population.As a Lead Health Home, CRHC is responsible for the quality oversight and management of the network of subcontracted agencies to perform their obligations to Health Home Members. This includes ensuring the agencies and their staff provide quality and timely Member care, as well as meet compliance metrics set forth by New York State Department of Health (NYS DOH). While CRHC is currently comprised of 15 agencies, which together have over 2000 staff and over 3,500 enrolled Members, the network continues to expand and additional quality support and oversight are required.The ideal candidate will be someone who exhibits compassion and understanding regarding the work the Health Home does to support a complex and vulnerable population with a myriad needs which may include, but are not limited to: medical diagnoses, psychiatric diagnoses, substance use disorders, financial stressors, housing instability, food insecurity, childcare needs, and lack of transportation. The ideal candidate will possess strong oral and written communication skills, specifically around the ability to interpret data into meaningful steps for quality improvement, as well as have strong attention to detail, strong organizational skills and strong computer skills, including the use of data management systems (electronic health record, database familiarity, Microsoft).Education Requirements :

Bachelor’s degree in Education, Human Services, Healthcare Administration or a related field required.

Master’s Degree preferred.

Experience Requirements :

Demonstrated competence and experience in the practice of quality management or quality improvement; including auditing records, trend identification and interpreting both qualitative and quantitative data into actionable steps needed to improve quality.

Health Home experience preferred.

Essential Job Functions and Responsibilities:Engages professionally with all staff, Members and community representatives as evidenced by manner of communication, dress, core values represented and quality of work. Performs audits of charts to support the formal annual review of each agency's performance (Quality Management Program - QMP). Performs additional audits as needed when cases come to the attention of the Lead Health Home or new program requirements are enacted. Provides quality support to CRHC's onsite Care Management Agency, Samaritan Care Management, including monthly spot audits and active participation in the team's monthly Quality Meeting. Reviews and analyzes data from QMP activities (Chart audits and aggregate data) as well as Member Satisfaction Surveys and prepares and presents reports interpreting data into meaningful action steps. Participates in CRHC's Quality Sub-committee on a quarterly basis. Works closely with Staff Development Sub-committee to secure training and educational resources in response to quality-related findings, which includes developing trainings based on needs identified via the QMP or other audits. Ability to work independently, setting priorities to coordinate care plan efficiently. As well as the ability to work in a team environmentPay Range - $31.50- $45.68Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
