Req Number 146038Job DescriptionReceives and processes patient referrals. Facilitates the paper flow of new referrals to ensure timely provision of services.Job Responsibility
Accepts referrals, processes them, and identifies referrals requiring RN review.
Secures all necessary demographic and physician information data; verifies and updates registration and agency as needed.
Completes enrollment documentation.
Initiates insurance verification form.
Maintains referral logs on daily basis.
Communicates with agencies on scheduling; i.e., specialty programs, geographic areas, etc.
Responds to requests/inquiries for private pay services, communicating information to appropriate office.
Obtains information from the system about patients requesting services after discharge from subacute or rehab facilities and communicates information through proper channels.
Contacts referred patients/family members to introduce services.

Job Qualification
Vocational or Technical school Diploma required, or equivalent combination of education and related experience.
Current License to practice as a Practical Nurse in New York State, and Current Basic Life Support Professional Certification required, plus specialized certifications as needed.
