We want everyone who works at CVS Health to feel empowered by the role they play in transforming our culture and accelerating our ability to innovate and deliver solutions to make health care more personal, convenient and affordable.Position SummaryThe Population Health Specialist for the Education System will support Plan Leadership in the design, implementation, and management of all PH and QI programs/initiatives related to the education system and improving outcomes for students enrolled in OhioRISE. The Population Health Specialist for the Education System will report to Population Health Director and serve as the subject matter expert on the education system for the OhioRISE Plan. This position will be responsible for working as a liaison between Aetna, the Ohio Department of Medicaid, and the Ohio Department of Education to ensure adherence to all education focused contract requirements, Plan priorities, and education directives as identified by the regulators. Additionally, this position, will work in collaboration with the Population Health Data Analyst to measure the effectiveness of education focused interventions and initiatives by leveraging data analysis to process, evaluate and make recommendations to meet objectives as they relate to education. This position is also responsible for coordinating the Plan's health equity activities specifically designed to focus on addressing health and education disparities among the OhioRISE population including high-risk and underserved, racial and ethnic minority populations and sexual and gender minorities.The Population Health Specialist for the Education System will also be responsible for supporting the ODM, MCE, and CME education focused strategies for population health and quality. The Population Health Specialist will foster and seek out learning and sharing of knowledge related to the education system and youth with behavioral health and multi system involvement. The Population Health Specialist will also support training and education for teachers, administrators, and other stakeholders on health promotion strategies, best practices, and relevant health topics. Deliver engaging presentations and workshops to build capacity and increase awareness of the OhioRISE program and education focused initiatives. Establish and/ or maintain collaborative relationships with all Ohio School Based Health Centers and all Educational Service Centers.The Population Health Specialist for the Education System will also be responsible for informing and supporting the Engagement and Outreach Team and the Communications Team to ensure Aetna OhioRISE is appropriately identifying and engaging with key stakeholders in the education system. Additionally this position will work collaboratively with the Family Engagement Director and the Youth Engagement Director to ensure the family voice and choice is utilized to inform all education focused activities and initiatives.Required Qualifications

Minimum 3 years' recent and related population health and/or public health experience.

Minimum of one year of recent and related experience working with local health departments, education systems, or managed care organizations.

Effective communication/influencing skills applied to broad internal/external audience.

Demonstrated proficiency with personal computer, keyboard and multi-systems navigation, and MS Office Suite applications.

Must reside in Ohio.

Ability to travel 25-30% of the time within the assigned region, with personal vehicle. Must possess reliable transportation, valid/active driver's license, and proof of vehicle insurance.

Preferred Qualifications

Strong knowledge of population health principles, epidemiology, health behavior theories, and health promotion strategies

Excellent communication and interpersonal skills to effectively collaborate with diverse stakeholders

Ability to design and implement health promotion programs

Knowledge of public health policies, laws, and regulations related to student health and well-being

Cultural competence and sensitivity when working with diverse populations

Strong organizational and project management skills to effectively manage multiple tasks and deadlines

EducationBachelors Degree required, Masters preferred.Pay RangeThe typical pay range for this role is:$46,988.00 - $91,800.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.In addition to your compensation, enjoy the rewards of an organization that puts our heart into caring for our colleagues and our communities.
