The older adult population is one of the fastest growing demographics in Central Oregon. The Council on Aging of Central Oregon (CoA) is responding to this increase in people 60 and older and their loved ones by adding a new Case Manager to work directly with clients who are part of the Oregon Project Independence Medicaid (OPI-M) program. OPI-M provides in-home services like housekeeping, personal care assistance, and Meals on Wheels to people who need assistance to continue living independently in their own homes. This is a rewarding position that will enable you to have an immediate impact on individuals, their families, and the community at large.

The Case Manager will perform assessments, design service plans, and advocate on behalf of clients in Crook, Deschutes, Jefferson, and N. Klamath Counties. This position will specialize in working with OPI-M clients as the program undergoes an historic expansion that will utilize federal funds to be able to serve more Oregonians. Unlike the other programs facilitated by CoA, OPI-M will support the senior population in addition to disabled adults 18+. Clients in this program will be shared with the local Aging and People with Disabilities office.

The OPI Case Manager reports directly to the Director of Client Services and works with a dedicated team who share a passion for advocating for older adults and those with disabilities living across the tri-county. This position offers a hybrid work model with the main office located in Bend. Travel throughout the tri-county area is required and a CoA company car can be used.

Hours

Monday thru Friday, 8:00 AM - 4:30 PM (40 hours per week)

Compensation

The salary for this position is between $25-30 per hour depending on experience as well as a complete benefits package, including health, dental and vision care; a 403(B) retirement plan, generous paid time off, an Employee Assistance Program, and more.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

Case Management:

-Develop and maintain a comprehensive understanding of Older Americans Act and Medicaid direct services

-Assess needs and eligibility for Oregon Project Independence (OPI) and OPI-M as well as other wraparound services for those clients including Meals on Wheels and Family Caregiver Support program.

-Monitor care/service plans for clients. Includes but is not limited to periodic plan reviews and reassessments.

-Work with in-home care providers and homecare workers to maintain service plans and meet the needs of the client.

-Work effectively with other social service agencies.

-Collaborate with the local APD office to determine services for clients on a shared caseload.

-Perform home visits including traveling in adverse weather conditions.

-Maintain digital records in a timely fashion.

Community Outreach:

-Outreach to the public at large and to low-income, isolated, and under-served diverse senior populations to promote services through tabling events, networking and information and referral.

-Participate in and facilitate community awareness with other local service providers including attending events and networking meetings.

Administrative

-Actively work with the case management team to achieve team goals and CoA objectives.

-Adhere to HIPPA and CoA confidentiality standards.

-Communicate client or staff feedback to supervisor.

-Conform to CoA policies, procedures, and protocols.

-Uphold the CoA mission, vision and values.

Minimum Qualifications (Knowledge, Skills, and Abilities)

-Bachelor’s degree in Human Service related field of study preferred or 3+ years of case management experience, preferably in geriatrics

-Bilingual (English and Spanish) is preferred, but not required

-Proficient in MS Office, web based applications and data entry

-Proficient in customer service and problem resolution

-Excellent written and oral communication skills

-Excellent organizational, planning, and prioritization skills

-Excellent interpersonal skills
