Position: Direct Care Professional
Overview: Special Education Assistant to provide support during the LEAD & Mentorship program workshop series and/or provide one-on-one support with participants with their coursework. The Special Education Assistant will also assist in adapting the learning materials based on participants needs.
Special Education Assistant Requirements:
General knowledge of the practices, methods and techniques used in the teaching of special education population;
ability to maintain files and information;
skill in the use of classroom and instructional equipment;
ability to operate audio visual, standard office, word and data processing equipment;
ability to establish and maintain effective working relationships with instructors, parents, participants, and administrators.
Direct support opportunity provides support, personal care, supervision, and training to individuals in their home and in the community. Each of our clients has unique support needs. Your support may include teaching daily living skills, performing personal care and household tasks, facilitating community integration and relationship building, as well as assisting with medication administration and managing behaviors.
Direct Care Professional requirements:
Coach, teach, and mentor people with intellectual disabilities
Assist and train clients with all areas of daily living skills
Implement Individual Support Plan programs, plans, and services
Transport and accompany clients to appointments, or other activities in the community
Skills and Abilities
Outgoing and friendly
Ability to provide accurate and clear written communications both electronically and verbally
Punctual and responsible
Ability to work with challenging individuals
Effectively communicate with management, peers, residents, and the public as needed
Able to work in a team and alone
Able to read and follow policies and procedures
Maintain great work habits.
Education & Experience
High School Diploma or GED required. 2 years’ experience working with individuals with disabilities, preferred. A comparable amount of training and experience may be substituted for the minimum qualifications.

Job Description #2:
Position: Community Inclusion Specialist
Overview: The Direct Care Professional plays a key role in supporting individuals with diverse abilities to actively participate and engage in community activities. This position involves providing direct care, fostering social connections, and promoting community inclusion for program participants.
Responsibilities:
1. Individual Support:
Providing direct care and support to individuals with diverse abilities in community settings.
Fostering independence and assisting with activities of daily living as needed.
2. Community Integration:
Planning and implementing community-based activities to enhance social inclusion.
Facilitating the participation in recreational, educational, and vocational activities.
3. Goal Setting:
Collaborating with the program participants to set personal goals related to community inclusion.
Supporting individuals in achieving their goals through encouragement and guidance.
4. Communication:
Maintaining an open and effective communication with program participants, their families, and support teams.
Acting as an advocate for individuals to ensure their needs and preferences are understood and respected.
5. Documentation:
Keeping an accurate and up-to-date records of participant progress, activities, and any incidents.
Preparing reports as required by organizational policies and regulatory standards.
6. Crisis Intervention:
Responding calmly and effectively in crisis situations, ensuring the safety and well-being of program participants.
Following the established protocols and procedures for crisis intervention.
Qualifications:
2 years’ experience in Community Inclusion
High school diploma or GED.
Valid driver's license, reliable vehicle and able to provide current insurance
Excellent interpersonal skills, a caring approach & physically fit
National Caregiver certification preferred
Candidates must successfully pass a background check and complete fingerprinting
CARD certified (OR equivalent)
Experience working with underserved marginalized communities
Reliable transportation is a requirement
