Description
The Direct Support Professional is responsible for safety and supervision of our consumers with Behavioral health histories. This role supports our consumers to develop healthy and socially acceptable behaviors while living in community-based sites. This position will coordinate with clinical staff, implement residential-based intervention, and assist the clients in achieving their clinical goals. The DSP is responsible for fulfilling the obligations as outlined in the client's program plan. The DSP will prompt or assist, as necessary, to ensure the clients participate in the activities of daily living, both in the home and the community. A commitment to the RHD values should be demonstrated as job duties are performed.
Job Responsibilities:
Provide supervision of individuals in accordance with the person-centered ISP; supporting someone to work towards outcomes identified in ISP and other Person-Centered Support Plans
Complete required documentationAdminister Medication
Provide transportation to consumersSupport consumers in activities of daily living, meal prep, toileting, hygiene, as appropriate
Assist in Community Integration Activities and access to community Supports including medical appointments, social security access, etc.
Complete all other miscellaneous tasks as assigned by supervisor
This Direct Support Professional role will be employed by Haldeman-Grant, a program within RHD. Haldeman-Grant is a residential program that provides the services needed for an individual to maintain independence in the community around them. Haldeman-Grant support's individuals who are Aging In Place and require support, medically, mentally, emotionally, and socially. Haldeman-Grant fosters a nurturing atmosphere of trust, support, independence, and safety. It is a program that facilitates growth by building trust between the staff and the program participants. Support services, at Haldeman-Grant, are person-centered and bi-lateral with an equal exchange between the participant and staff. The focus remains on wellness and recovery.Open Position: Part-time ( On-call ), flexible schedule.
Requirements
High School diploma or GED is required, associates or bachelor's degree is preferred
Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job.
Prior experience in the human services field, preferred.
Valid driver's license
Subject to employment screenings, including FBI clearance. if applicable
