Position: Health Management CoordinatorLocation: Columbia, SCDuration: 4+ monthsPay Rate: $23/hr on W2We are looking for a Health Management Coordinator to join one of our Fortune 500 Healthcare clients.Job Summary :The Health Management Coordinator assists in implementing health management programs for members, focusing on high-risk individuals. This role involves coordinating programs, providing health coaching, analyzing reports, and identifying areas for program improvement.Key Responsibilities :
Program Implementation & Coordination (50%) :
Coordinate and facilitate health management programs for members, focusing on high-risk individuals.
Analyze reports and update procedures as needed.
Strategy Development & Communication (25%) :
Assist in developing strategies to increase program participation and effectiveness.
Maintain communication with provider networks and members.
Health Coaching (15%) :
Provide telephonic or in-person coaching for assigned health programs.
Document patient interactions using data systems.
Program Improvement & Development (10%) :
Identify areas for health program development and implement improvements.
Create educational materials for patients and providers.
Required Skills :
Strong organizational, analytical, and communication skills.
Ability to work independently and make sound decisions.
Discretion in handling confidential information.
Proficient in Microsoft Office and health management data systems.
Education & Experience :
Required: Bachelor’s degree in Nursing, Health Administration, Health Promotion, Exercise Science, or related field.
Preferred: 1 year of clinical experience or 2 years in health administration.
Preferred: Knowledge of HMO operations and quality improvement processes.
