The Case Manager is responsible for performing patient care activities as directed by the program manager, orienting, supporting discharge planning from intake to discharge, building rapport with referral sources, and connecting clients to community resources. The Case Manager’s responsibility is to provide direct support and resources to the client and clinical staff.  This position is 100% remote. ResponsibilitiesSupport Discharge Planning nationwide with Virtual IOP Programming (CA, CT, TX, MD, FL)Actively participates in Multidisciplinary Treatment Team meetingsServes as the liaison between the clinical and Outreach department  Facilitates psychoeducational groups at the directive of the program manager Shares pertinent information with team as obtained from client’s family, or others as indicated throughout the program.  Recognizes, responds to and assists in psychiatric crisis situations and provide the intervention necessaryDevelop resources to provide clinical and clients in order to meet client needs after treatmentCommunicates effectively with client and interdisciplinary team regarding client needs Assists with chart documentation and compliance For a virtual tour of the facility, please visit our website at discoverymood.com
