The NP will be responsible for providing advanced nursing care to patients in a clinical setting under the direction of a physician. This individual will educate students and patients about chronic disease management and provide resources and support to manage conditions effectively. Also assists in the development and implementation of clinic protocols, guidelines, and collaborative service efforts. Experience with managing hypertension, obesity, and uncontrolled diabetes is a plus!This position is funded by grants that comprise 50% or more of the funding. Continued employment in this position is contingent upon the availability of funding for this position. Position Summary:Provides advanced nursing care to patients in a clinical setting in an area of specialization under the direction of a physician.Position Key Accountabilities:
Performs relevant patient assessments and physical examinations.
Coordinates, directs, and implements patient’s plan of care.
Orders, conducts, and interprets pertinent laboratory and diagnostic studies and procedures.
Makes medical recommendations and/or referrals.
Formulates, implements, and evaluates an outcome-based treatment plan.
Maintains appropriate records detailing the patient’s treatment plans and outcomes.
Facilitates patient education relating to medical processes, medication, nutrition, and health promotion.
Collaborates with patients, families, other clinicians, and referral services to assist in the patient’s recovery.
Other duties as assigned.
