At Houston Methodist, the Population Health Advisor position is responsible for understanding the programs and initiatives being deployed by Houston Methodist Coordinated Care, identifying practice improvement opportunities and integrating clinical programs and resources into the practice workflows with the intent to optimize the patient experience, quality of care provided, and positively impact resource utilization. This position supports the physician practices by analyzing performance reports and data to inform decision-making, process, and program implementation. The Population Health Advisor position monitors and implements educational and process interventions based on practice-level data, trends and identified opportunities.PEOPLE ESSENTIAL FUNCTIONS
Creates and maintains relationships with assigned primary care practice physicians and staff, high value specialists, urgent care centers, and other community stakeholders.
Advises primary care practices, physicians, nurses and other clinical staff to assist them on their conversion to value-based care.
Provides educational and training sessions related to Houston Methodist Coordinated Care programs and initiatives which include topics related to quality, utilization, and patient satisfaction.
SERVICE ESSENTIAL FUNCTIONS
Travels throughout the Houston, TX area to meet with physician practices to support clinical practice transformation.
Communicates and supports payment methodology transition(s) e.g., education & review of compensation packages.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
Identifies process improvement opportunities in Houston Methodist Coordinated Care physician practices.
Provides reporting on primary care practice quality metric performance which includes the dissemination of ¿gap closure¿ lists and best practices to promote continuous improvement.
FINANCE ESSENTIAL FUNCTIONS
Disseminates quality and efficiency reports to Houston Methodist Coordinated Care physician practices.
Analyzes reports for practice specific trends and provide guidance and ongoing support to primary care physicians and practice staff for continual process improvement.
GROWTH/INNOVATION ESSENTIAL FUNCTIONS
Tests and refines workflows that support sustainable transformation, and guide practices as they expand interventions to additional patient populations.
Collects best practices related to quality and efficiency initiatives and disseminates best practices to other practices and Population Health Advisors.

Master's degree in applicable field preferred
