Our collaboration is focused on increasing access to care through a growing network of leading specialists and revolutionizing healthcare to save lives and improve the health of the communities we serve.ResponsibilitiesThis position serves as a liaison between the Transplant Nurse Coordinator, Transplant Physicians, hospital ancillary departments and referral sources for patients who are referred for transplant and/or VAD services. This position works in partnership with the nurse coordinator to expedite the referral and evaluation process for potential transplant candidates and/or coordinates important post-operative care appointments. This position also provides organizational support for pre and post transplantpatients by ensuring receipt of test results, facilitating correspondence, appropriately triaging patient phone calls and maintain records.ESSENTIAL KEY JOB RESPONSIBILITIES
Coordinates the patient evaluation process for transplant candidates by facilitating referrals, insurance verification, obtaining test results, scheduling appointments and ensuring timely communication is provided to all team members including the patients during the entire process.
Tracks and schedules follow-up procedures and appointments
Prepares letters, reports and correspondence in a correct, businesslike manner. Prepares materials for the multidisciplinary Transplant Medical Review Board, including follow up correspondence to patients, referring physicians and insurance case managers. Ensures that patients receive the appropriate educational materials for their transplant appointments.
Initiates and follows through on pre-certification requirements, referrals and communication with patient's insurance company/PCP.
Sorts mail on a daily basis, initiating action when appropriate (i.e., lab results, patient request for information, physician correspondence).
Organizes and maintains patient charts, filing, obtaining correspondence and test results, and sorting results which need physician review prior to filing.
Obtains and communicates clinical information from patients, families, medical record and other members of the health care team to nurse coordinators
Obtains data and/or other specific information as requested by the Transplant Nurse Coordinators for submission to UNOS
Provides financial aid application for patients needing medication and medical care assistance.
Demonstrates accountability for patient safety and reporting concerns in an accurate and timely manner to members of the team.
Coordinates patient clinic experience and assists in facilitation of actual clinic visit.
Assists with office supply inventory and ordering under the direction of department leadership.
QualificationsRequired Education and Experience:
High School Diploma/GED
Two (2) years of relevant experience
Required Minimum Knowledge, Skills, Abilities, and Training:
Computer skills required including applications such as Excel, Word and Outlook.
Experience in a health care setting required and familiar with medical terminology.
Requires excellent communication skills with members of the health care team, community facilities and vendors, and most important, patients and their family members.
Demonstrates ability at self-direction, patient advocacy, follow through, and evaluation of services rendered.
