Director Of Nursing (DON) / Director Of Clinical Services (DCS)
Perform Competency evaluations of staff
Supervision of clinical staff
Participation in disciplinary decisions and actions
Incidents to report to ACHC (license suspensions, conditions/restrictions to licenses, open investigations to any
regulatory or governmental authority)
Establishing and/or evaluating appropriate performance standards in accordance with CMS, requirements to meet
Medicare Conditions of Participation and ACHC guidelines
Training of staff on home health care policies and procedures, including in-service training
Directing/training clinical employees to complete various duties, annual competency evaluation of all clinical
employees
New hire Orientation per policy and procedures
Performing assessments and reassessments using OASIS
Scheduling & performing in-home assessments, making patient and personnel assignments, clinical
management for in-home visits and adequate staffing
Performing supervisory visits to evaluate work and ensuring staff is providing quality care/services per
Individualized Plan Of Care
Evaluating effectiveness of care provided by RN, LVN, CAN, PT/PTA, OT/COTA, ST, MSW, Caregiver
Coordination of Care for patients
Coordinating referrals/intake
Individualized Plan Of Care for patients
Admitting, transferring, recertifying, discharging patients
Coordination of DME supplies and ancillary services for patients
Developing, revising, and evaluating home health care service goals/budget
Quality Assurance (QA) reviewing data, assisting and instructing staff with completing data analysis and reports
accurately and completely including electronic reports of all OASIS Data collected from assessments
Quality Assurance and Performance Improvement (QAPI) Compliance with CMS, ACHC, quarterly audits, on-
going audits, discharged patient reports, leading QAPI Committees, monitor and advise quality improvement against
goals/objectives
Participating in and leading a variety of meetings and committees to receive and convey information
Knowledgeable in all aspects of field case management utilized for home health
Effectively organizing, planning, and directing clinical department functions and activities
Being available at all times during working hours
Consistently ensure compliance with all established standards of Nursing Practice
Participating/facilitating Professional Advisory Board Meetings/Governing Body meetings
