The LVN will perform home health visits for an established agency. We are a Medicare-certified home health care organization; we are one of the fastest growing home care companies with a passion for its employees and patients. This position is an excellent opportunity for someone with great clinical skills, exceptional patient care and communication skills.The LVN's primary function is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency.RequirementsJob Qualifications:Education: Graduate of an accredited School of Vocational/Practical Nursing.Licensure: Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC, current driver’s license.Experience: Two years of experience as a Licensed Vocational/Practical Nurse in a clinical care setting, Home Health, geriatric or med surg preferredSkills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.Transportation: Reliable transportation. Valid and current auto liability insurance.Environmental and Working Conditions:Works in patient’s home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather.Physical and Mental Effort:Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.
