SUMMARYThe primary purpose of the Nurse Navigator position is to coordinate and implement patient care across the continuum of care. The Nurse Navigator is accountable and responsible for providing quality, evidence-based patient care through the appropriate use of resources in collaboration with the inter-professional team. Acts as a patient and family advocate to uphold patient rights. Organizes and prioritizes work of patient care teams; trains, coaches and assists team members to ensure optimum patient care and efficient use of resources.SHIFT Monday - Friday 8:00 a.m. - 5:00 p.m.MISSION STATEMENTThe mission of The University of Texas M. Participates in daily unit operations by working with the breast screening program to identify patients with abnormal screen findings and offer MDACC service for further workup. Assists with triaging such patients to Cancer Prevention Undiagnosed Breast Clinic for further assessment.b. Facilitates the entry of patients into the breast cancer program by reviewing breast imaging results and medical history indicative of newly diagnosed breast cancer patients to achieve and maintain the financial goals by utilizing all resources effectively and efficiently.c. Triages and refers newly diagnosed breast cancer cases to appropriate service for further care coordination. Obtains medical information to facilitate initial consultation.b. Practices autonomously within the scope of professional nursing practice - patient care competencyc. Participates in change initiativesd. Assists clinic staff, research nurses and physicians with accruing eligible cancer patients to clinical trials.h. Actively involves the patient/family in the plan of care and conducts patient/family education as appropriate.i. Develops relationships with all MDACC breast program physicians and serves as a liaison between MDACC physicians and patient's referring physician.j. Provide pre/post biopsy education and patient support, pathology follow up and completion of patient/physician communication and ensuring patient consent obtained prior to procedure.Empirical Outcomes: The Nurse Navigator is responsible for seeking out and maintaining knowledge regarding relevant empirical outcomes, including the impact of quality improvement initiatives and institutional best practices on the patient population, nursing workforce and institution, as reflected in unit and institutional metrics.a. Assists with tracking of metrics and quality indicators for diagnosing and treating cancers promptly and effectively.b. Becomes proficient in and utilizes MDACC patient care records for documentation of care. Provides reports and organizes data as requested.Other duties as assigned.Required education: Bachelor's degree in Nursing.Required experience: Five years professional nursing experience to include three years oncology nursing. With preferred degree, two years Advanced Practice Registered Nurse experience in an oncology setting.Required certification: Current State of Texas Professional Nursing License (RN). Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
