POSITION LOCATION
This is an on-site case management role based in Plainfield, VT, within a 24/7 residential program. The position will involve providing case management activities throughout Washington County, primarily focusing on the Barre and Montpelier areas. Occasional travel outside the county may also be required.
POSITION SUMMARY
Elevate Youth Services (EYS), a private non-profit established in 1974, is the only organization in Washington County dedicated solely to addressing the complex needs of adolescents and young adults. Serving nearly 500 youth annually through its direct service programs, EYS also leads two statewide coalitions that impact an additional 1,000 youth across Vermont. Guided by a mission to promote safety, competence, and confidence, EYS supports youth on their journey through adolescence to adulthood.
The Homeless Youth Programs Case Manager provides engagement and case management services to youth ages 18-24 who are living in Elevate Youth Service’s HOP House program. Caseload will include approximately 8-10 youth. Responsibilities include intake and assessment; individual and group counseling; crisis intervention; case planning; resource brokerage and referral; case coordination and advocacy; follow-up and after care; timely provision of clinical and financial record keeping related to job duties. Additional case management may be provided for other youth aged 16 through 24 who are homeless or at risk of becoming homeless but are not immediately housed by EYS.
The position works collaboratively with Elevate Youth Services’ additional homeless youth case managers to ensure agency representation in region-wide homelessness efforts including representing EYS on the Washington County Continuum of Care. This position requires direct one-on-one case management with program youth while longer-term solutions to allay homelessness are identified and pursued.
The ideal candidate will be empathetic, caring, organized, have strong relationship-building skills and will serve as an effective advocate for youth served.
Individual Case Management Duties Include:
Maintain an active and positive presence within the HOP House
Develop strong relationships with residents through regular check-ins, room visits, and outings
Provide crisis prevention, intervention, and safety planning services, as well as stress management and life skills development
Assist young people in applying for and obtaining economic benefits, housing, and employment, and connect them with outside agencies and supportive services that promote their health and wellness
o Help youth apply for all appropriate mainstream benefits, including health insurance, 3SquaresVT, WIC, Reach Up, SSI/SSDI, etc.
o Assist youth in identifying housing options, applying for subsidized housing, securing lease agreements with HUD voucher support or placing youth in one of the Elevate’s Master-leased transitional Apartments
Facilitate transportation and provide accompaniment, as needed, to client appointments in the community
Work closely with community providers and internal service teams to ensure coordinated and effective services
Maintain accurate, timely, and detailed progress records and complete necessary electronic data entry into agency databases
o Utilize program application to screen for appropriateness for programming and explore diversion options with each client before offering them housing placement.
o Complete initial treatment plan with youth and maintain files during the term or length of services provided that meet regulatory and funder compliance.
o Conduct required intakes, evidence-based assessments, and coordinated entry assessments with youth entering the program and support youth in completing all available housing applications.
o Use required screening tools to assess need for substance abuse/use services, behavioral/emotional counseling services, AA / NA support groups, DV services, employment services and job training, educational supports, and medical/dental services for each youth that enters the program and make referrals as needed.
Identify natural supports for youth and a plan to access them, as appropriate.
Participate in agency staff meetings and professional development activities.
QUALIFICATIONS:
1. Understanding and Knowledge
o In-depth understanding of issues related to poverty, substance abuse, mental health, trauma, abuse, adolescent development, and the social rights of young people.
o Knowledge of service systems and clinical issues affecting out-of-home youth and young adults.
o Proficiency in positive youth development principles and their application to working with young people.
2. Experience and Commitment
o Demonstrated experience working with LGBTQIA+ individuals, culturally diverse staff, participants, and communities.
o At least two years of experience working with youth aged 18–24.
o Proven ability to develop and implement high-quality, individualized case management services tailored to the developmental needs of youth.
3. Skills and Abilities
o Exceptional communication, organizational, creativity, flexibility, optimism, and interpersonal skills.
o Strong advocacy, engagement, and collaboration abilities to support clients effectively.
o Comfort and flexibility providing services in non-traditional settings (e.g., homes, community spaces, cafes, schools, parks).
o Proficiency with computer applications, databases, and Microsoft Office Suite (including spreadsheets).
o Strong technical skills, including the ability to use modern tools and relevant technology.

5. Education and Certifications
o Bachelor’s degree in a related field from an accredited college or university required; Master’s degree preferred.
6. Physical Requirements:
o This role involves navigating stairs and occasionally lifting and carrying items up to 25-35 lbs. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position.
7. Other Requirements:
o A valid driver’s license with a clean driving record.
o Successful completion of a background check.
WORKING ENVIRONMENT:
HOP Houses are a fast-paced, dynamic, residential environment. The program participants experience successes and challenges on a daily basis. The program offers many opportunities for creative program development, while maintaining appropriate accountability for the participants. Some evening and weekend work, including on-call duties will be required.
EYS provides direct services to youth and families in Washington County, as well as administration of several statewide programs. Its mission is to promote safety, competence, and confidence as youth create their path through adolescence and into adulthood. All EYS employees attend a staff meeting once a month, as well as other staff development events throughout the year.
