See below for additional information.The Home Care RN Clinician 2 is an experienced capable clinician focused on expanding knowledge and skills. Consistently provides effective direct care to patients in their homes for an assigned caseload of patients; as part of the interdisciplinary team; oversees the care planning and care coordination for a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients their families Physicians and other members of the health care team and in compliance with homecare standards and regulatory requirements. Seeks as well as provides feedback for improved clinical practice. Assumes a beginning leadership role but seeks mentoring in this process.

Position Compensation Range: $54,537.60 - $90,272.00 AnnualMINIMUM REQUIREMENTSEducation: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.Experience: 1 year of Relevant ExperienceLicensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver’s License required. American Heart Association (AHA) Health Care Provider BLS certification required.PHYSICAL DEMANDSJob may require sitting for prolonged periods, frequently standing, walking, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.ADDITIONAL INFORMATIONThis position qualifies for the UVA Medical Center Clinical Ladder. The salary range reflected in this job posting is for an RN Clinician 2. Additional compensation is available if you are hired into a higher level on the Clinical Ladder. Those hired as either a Clinician 3 or Clinician 4 must have a Bachelor of Science in Nursing Degree and possess a specialty certification from a national certifying organization and meet applicable Career Ladder Behaviors. Placement on the clinical ladder will be discussed during the recruitment process. Current team members already working as RN Clinician 3 or 4 who wish to transfer to another department will have their level on the clinical ladder reviewed by the hiring manager and the PNSO if applicable.This position would be ideal for a current RN that works primarily at the hospital – providing teaching to hospitalized patients who are discharging home with infusion therapy or enteral therapy. The RN teaches the patients how to hook up their infusion medications or tube feedings via a pump (or eclipse or syringe.) The position does have an on-call component, that includes after hours and weekend on-call (usually once every 4-6 weeks, for a week at a time.) Usually the infusion on-call RN has a maximum of 1 or less calls per night, and teaches 1-3 patients at the hospital on weekends when it is their on-call weekend. This is a terrific role for nurses who like to teach and they become a source of support for hospital case managers and discharge planners as they help patients transition home.
