Primary Function: (OVERVIEW)The University of Virginia Physicians Group (UPG) seeks a dedicated Family Nurse Practitioner for the Department of Medicine : Division of Palliative Care working in our outpatient practice. University Physicians Group provides comprehensive outpatient and inpatient care to patients within the Department of Medicine: Division of General Medicine, Geriatrics and Palliative Care. Care includes evaluation and management of acute and chronic illnesses, health maintenance and disease prevention. This is a full time position with the Division of Palliative Care, with practice hours Monday-Friday. We offer a highly competitive salary, and benefit plan.Our practices are affiliated with UVA Medical Center- widely recognized for its quality of care, the #1 Hospital in Virginia for 2017, according to U.S. News and World Report.PRINCIPAL DUTIES AND RESPONSIBILITIES : ClinicalNurse Practitioner

Assesses and documents patient and family response to intervention

Demonstrates ability to carry out critical skill as outlined in procedures and specific standards

Identifies each patient prior to administering medications and providing care and treatment.

Performs technical skills and administers medications in a safe and efficient manner.

Establishes priorities based on patient needs and emergent care

Provides supervision and direction to other team members. Delegates task according to capabilities and scope of responsibilities.

Ability to independently review patient problems including, but not limited to medication refills, determining the need of follow up care and referring appropriately to the physician according to policy.

Performs all prescribed laboratory test, sends to appropriate laboratory, receives and interprets laboratory values, and notifies physician of results

Providing patient education and counseling to patients and their families

Participate in chronic disease management of patients

Manage patients chronic disease

Provide diabetic disease management to patients

Performs assigned work safely, adhering to established departmental safety rules and practices.

Reports to the appropriate personnel, in a timely fashion, any unsafe activities, hazards, or safety violations that may cause injury to oneself, other employees, patients, and visitors.

Demonstrates skills and knowledge necessary to provide care appropriate to (neonatal, pediatric, adolescent, adult and geriatric patients), including knowledge of growth and development, that ability to obtain and interpret information to identify patient needs, and

provide the care needed.

Perform other responsibilities as assigned, and performs designated procedures, which are included in the written agreement established with the collaborating attending’s (s) as documented in the Credentialing practice agreement.

Master’s Degree as an Advance Practice Provider ( Nurse Practitioner )

License or license eligible as an Advance Practice Provider (Nurse Practitioner

Must demonstrate and maintain all credentialing and licensing requirements of the UVA Medical Center,Nurse Practitioner ;the appropriate certifying body

Current Basic Life Support (BLS) required

Current PALS as required specific to position

Advanced Cardiac Life Support (ACLS) as required

Experience:

Prefer 1-2years minimum experience as an Advance Practice Provider : Nurse Practitioner

  • The Advanced Practice Provider Manager is a clinical expert and an administrative leader who manages Advanced Practice Provider (APP) team members within their respective role in provision of the organization, service line, and departmental missions. This role is responsible for the day-to-day operations of APPs in patient care, research, education, professionalism, and regulatory compliance. The APP Manager is accountable for human resource needs, workforce planning, development of staffing models, orientation, onboarding, professional development, employee engagement and satisfaction, budget planning, and partnership between clinical service / service line and UVA Health leadership. The APP Manager works closely with service line Chiefs and Medical Directors to assure effective and timely implementation of operational initiatives. The APP Manager partners with other UVA Health leaders to achieve common goals. Additionally, the APP Manager has direct responsibility for the APPs across the continuum, integrating the boundaries of setting or entity. The APP Manager has exemplary clinical practice and devotes time to both consistent clinical practice duties and administrative work.
