POSITION SUMMARY
The Live-In Supportive Care Provider is responsible for providing 24-hour supportive supervision and daily living assistance to four residents in a supportive housing environment. The provider ensures clients’ safety, wellness, medication compliance, cleanliness, and engagement in daily routines. This role requires compassion, strong organizational skills, and the ability to manage multiple care needs within a home-like setting.

KEY RESPONSIBILITIES
1. Direct Client Care
Provide daily supervision and support to four clients.
Assist residents with personal care needs as appropriate.
Monitor clients for safety, wellbeing, and behavioral changes.
2. Medication Management
Administer medications as directed.
Maintain accurate medication logs and documentation.
Track refill dates and coordinate with pharmacies and providers.
3. Household Management
Prepare and cook daily meals for residents.
Perform routine cleaning, laundry, and household organization.
Maintain sanitary living conditions in all common and private areas.
4. Transportation & Appointments
Accompany clients to doctor’s appointments, therapy, and community activities.
Ensure appointment follow-up, paperwork, and coordination with providers.
5. Documentation & Reporting
Complete daily progress notes for each client.
Maintain required forms, logs, and incident reports.
Report changes in health, behavior, or safety concerns immediately.
6. Monitoring & Support
Support residents with routines, chores, and daily structure.
Encourage goal-setting, independence, and healthy habits.
Provide crisis de-escalation and redirection when needed.

QUALIFICATIONS
Experience working with individuals with mental health, developmental disabilities, or medical conditions.
Strong communication, time management, and organizational skills.
Ability to work independently with limited supervision.
CPR/First Aid (preferred).

The salary is based on experience, responsibility level, and number of clients served.
Entry Level: $35,000–$42,000
Experienced Provider: $42,000–$52,000
Live-In providers caring for 4 residents with medical/behavioral needs typically fall at the higher end of the range.
