Hope Center Case Manager

Job Title: Case Manager – Hope at Work Program

Location: The Hope Center

Position Type: Part-Time or Full-Time, starting at $45,00-$50,000 for full-time (Dependent on Experience)

Hours: Monday - Friday, between 8:00 am - 6:00 pm

About The Hope Center:

The HOPE Center is a Christ-centered ministry dedicated to helping individuals make positive life changes through workforce training, personal growth, and spiritual support. We provide individuals with the tools they need to succeed in the workplace, build personal growth, and overcome life's challenges. The Case Manager will offer holistic, individualized support, including Biblical Counseling using the Genesis Process Relapse Prevention Curriculum, to participants in our program.

Role Overview:

This position is specifically for a female to work with the women in our program. The Case Manager will support and guide participants in the Hope at Work Program, helping them achieve personal and professional goals. Responsibilities include conducting assessments, creating individualized plans, and offering Biblical Counseling (Training provided) to help participants navigate challenges, both emotional and spiritual. The Case Manager will work closely with job coaches and other staff to ensure participants receive the necessary resources and support to succeed.

Key Responsibilities:

Conduct assessments and develop personalized action plans for each participant.

Provide Biblical Counseling using the Genesis Process Relapse Prevention Curriculum to help participants overcome personal struggles and identify areas for growth. (Training provided)

Offer one-on-one support and guidance, fostering personal development and faith-based growth.

Assist participants in developing life and relational skills essential for success.

Coordinate resources such as housing, healthcare, and community services for participants.

Regularly track progress and provide feedback to participants, adjusting plans as needed.

Provide crisis intervention and conflict resolution when challenges arise.

Collaborate with job coaches and staff to maintain a supportive, coordinated program environment.

Maintain accurate case files and prepare reports as needed.

Integrate Christian values and biblical principles into case management when appropriate.

Additional Responsibilities May Include:

Networking & Partnerships: local law enforcement, probation officers, court systems, and other community organizations.

Desired Qualifications:

This position is specifically for a female to work with the women in our program.

Growing Christian with a heart for service and ministry.

Active involvement in a Bible-based Christian church.

Experience in case management or related fields (addiction and relapse prevention experience preferred).

Certification or experience in the Genesis Process Relapse Prevention Curriculum is highly desirable.

Strong interpersonal skills and the ability to build rapport with diverse individuals.

Ability to manage conflict with professionalism and respect.

Knowledge of community resources and social services.

Ability to offer support to help individuals confront and overcome emotional and spiritual challenges.

Strong written and verbal communication skills.

Ability to work collaboratively and professionally in a team environment.
