Description

SCHEDULE: 1.0 FTE; averaging 40 hours per week. Monday-Friday, 8a-4:30p PST. ONE WEEKEND per month will be required to work.

Supporting Case Managers on acute/inpatient unit. Will be patient facing.

The case management associate participates on the Case Management Team (CMT), with the guidance of the nurse case manager and social worker, assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with Medical staff, CMT members, Business Office, Medical Records and other hospital staff; performs other duties as assigned.

Required Qualifications:

1 year Job related experience in a healthcare setting

Preferred Qualifications:

Associate's Degree

Bachelor's Degree

Upon hire: Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology

Prior experience in discharge planning, billing, utilization review, financial counseling or registration

Proficiency in medical terminology
