Coordinates discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agencies.
Performs a psycho/social assessment for continuity of care planning for in/outpatients.
Provides short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability.
Participates as a member of an interdisciplinary team to develop a coordinated care plan for patients.
Participates in quality assurance and utilization review to enhance patient satisfaction.
Participates in informal and formal consultations; provide, present, and participate in support and training; arrange speakers and develop materials and programs.
Participates in Medical Center committees and special projects as assigned.

We deliver inspired people to do meaningful work.Qualifications· Master's of Social Work from an accredited college or university is required.· Registration with the state of Washington as either a Licensed Social Work Associate (LSWAIC), Independent Clinical Social Worker (LICSW), Social Work Associate Advanced (LSWAA) or Social Work Advanced (LISW).
